Strengthen Community-based Care to minimize health inequalities and improve the integration of vulnerable migrants and refugees into local communities

Duration: 01/05/2017 – 30/04/2020 (36 months)

The project will:

o   Describe the current health profile of vulnerable migrants and asylum seekers as well as the needs, expectations and capacities of health service providers;

o   Develop a comprehensive toolbox for the implementation of community based care models including prediction models, best practice examples, algorithms and tailor-made materials;

o   Pilot test and assess community care models and produce guidelines and recommendations.
